What is eclampsia?
a severe complication of preeclampsia
It’s a rare but serious condition where high blood pressure results in seizures during pregnancy
What conditions must be met for eclampsia to be considered?
Convulsions in pregnancy or post birth (more than 20 weeks)
A pregnant/post birth patient has a seizure, they do have a history of preeclampsia/hypertension, what are your next steps?
Turn on lateral position (on their side)
Obtain IV access w/ 16 gauge
DO NOT give fluids
O2 sats titrate to 94-98%
Why should fluids not be given to a patients with pre-eclampsia?
Intravenous fluid given to women with pre-eclampsia may increase their risk of developing pulmonary oedema
A patient with a history of preeclampsia/hypertension is having convulsions lasting longer that 2-3 minutes and or has had a second convulsion, what are your next steps?
Administer magnesium sulphate 4g IV over 10 minutes or diazepam IV/rectal
A pregnant/post birth patient has a seizure, they have a history of epilepsy, what are your next steps?
Turn on lateral position (on their side)
Obtain IV access w/ 16 gauge
DO NOT give fluids

Maternal safety
How do you determine the transfer of an eclampsia patient?
Transfer to the nearest appropriate destination as agreed locally
Consider the specific clinical situation and which interventions may be required for the woman and baby on arrival
What additional actions are needed during the transfer of an eclampsia patient?
(Not medical based)
Pre-alert stating obstetric emergency eclampsia
Take maternity notes if available
