Advanced Life Support (Child) Flashcards
What are the stages of ALS (child)?
Unresponsive and not breathing normally
CPR 15:2 following 5 initial breaths, attach defibrillator/monitor, minimise interruptions
Assess rhythm
What is the process if a shockable (VF/pulseless VT) rhythm is presented?
1 shock at 4J kg^-1
Immediately resume CPR for 2 mins, minimising interruptions
Reassess rhythm
What is the process if a non shockable (PEA/Asystole) rhythm is found?
Immediately resume CPR for 2 mins, minimising interruptions
Reassess rhythm
What do you do if return of spontaneous circulation (ROSC) occurs?
Immediate post cardiac arrest treatment
What is involved in post cardiac arrest treatment?
Use ABCDE approach Controlled oxygenation and ventilation Investigations Treat precipitating cause Temperature control
What drugs should be given during paediatric CPR?
Give adrenaline every 3-5 minutes
Give amiodarone after 3rd and 5th shock
Things that must happen during ALS (child)?
Ensure high quality CPR Plan actions before interrupting CPR Give oxygen Vascular access (IV, IO) Give adrenaline every 3-5 minutes Consider advances airway and capnography Continuous chest compressions when advanced airway in place Correct reversible causes Consider amiodarone after 3 and 5 shocks
What does high quality CPR include?
Efficient…
Rate, depth and recoil
Possible reversible causes of cardiac arrest in children?
Hypoxia Hypovolaemia Hyper/hypokalaemia, metabolic Hypothermia Thrombosis (coronary or pulmonary) Tension pneumothorax Tamponade (cardiac) Toxic/therapeutic disturbances
Hypoxia?
Inadequate levels of oxygen in the tissues
Thrombosis?
Inappropriate, pathological formation of stationary blood clots within blood vessels
Hypothermia?
Abnormally low body temperature (core temperature <35 Celsius)
