What is oro-pharyngeal dysphagia?
Patient struggles to get food to leave the mouth
What can cause oro-pharyngeal dysphagia?
Problems coordinating muscles as a result of neurological disease
- stroke
- nerve palsy
What examinations should you do with oro-pharyngeal dysphagia?
Cranial nerve
Swallow safety
What is oesophageal dysphagia?
Food/fluid leaves mouth ok and then gets stuck
What can cause oesophageal dysphagia?
Obstruction
Neuromuscular problem
What is odynophagia?
Painful swallowing
What is globus pharyngis?
Persistent painless sensation that there is something stuck in the throat
What are causes of dysphagia?
Oesophageal cancer Oesphagitis Oesophageal candidiasis Achalasia Pharyngeal pouch Systemic sclerosis Myasthenia gravis Stroke
What is achalasia?
Failure of the lower oesophageal stricture to relax and allow food to pass
What are the symptoms of achalasia?
Difficulty swallowing - solids + liquids
Reurgitation
Heartburn
Weight loss
What are the signs on barium swallow of achalasia?
Bird beak sign
Oesophageal dilatation
How is achalasia managed?
Medication
- nitrates
- nifedipine
Balloon dilatation
Botox injection
Surgery
What investigations do you do for suspected dysphagia?
Bloods
- FBC
- U+Es
CXR
Barium swallow
Upper GI endoscopy
