what is Dysmenorrhoea
• pain + cramping during the menstrual cycle that interferes with normal daily activities
how is it classified
Primary or secondary
what is Primary dysmenorrhea
no obvious cause, may be due to ↑ prostaglandins
what is secondary dysmenorrhoea
due to endometriosis, fibroids, cervical stenosis or pelvic adhesions
how does primary dysmenorrhea presents as
crampy pain usually occurring on the 1sr or 2nd day of period
associated symptoms in primary dysmenorrhea
N+V, headache
what is seen on examination in primary dysmenorrhoea
no abnormalities but generalized tenderness in the lower abdomen/pelvis
symptoms of secondary dysmenorrhoea
secondary dysmenorrhea due to cervical stenosis presents with scanty menses and severe cramping that is relieved with increased menstrual flow
what is seen on examination of secondary dysmenorrhoea
there may be scarring of the external OS
what is indicative of dysmenorrhoea 2/2 pelvic adhesions
PMH of pelvic infections or pelvic surgical hx
how is diagnosis made
Hx and lack of organic cause
treatment
NSAIDs and oral contraceptive
treatment of secondary dysmenorrhoea
; treat underlying cause
Endometriosis = oral contraceptive, danazol or surgery
cervical stenosis = cervical dilation
pelvic adhesions = NSAIDs & oral contraceptive or surgery to lyse adhesions
