1 gm (1000 mg) over 15 minutes
for patients greater than 50 kg

.3 mg of Epi 1:1,000 for anaphylaxis

Initial ‐ 25‐50mcg IV/IO, 50‐100mcg IM, 100mcg IN
May repeat until 200mcg
Glucagon Dose
1mg IM (last resort if unresponsive)
Ketamine Dose
50‐69 kg – 15mg IV
70‐89 kg – 20 mg IV
≥ 90 kg – 30 mg IV
Lidocaine
40mg IO for pain due to IO
Midazolam
AKA Versed. For seizures, psych, and sedation for pacing, cardioversion, or advanced airway
Naloxone Dose
Overdose and respiratory depression: 2mg IN (1 mg each nostril) or 1‐2mg IV/IM
