What is DLE?
Dis. in which skin manifestation → may mimic SLE but systemic manifestations are rare
Skin lesions seen at?
Face, scalp & external ears
Describe the lesion?
Well demarcated
Discoid/ annular
Erythematous plaque
With adherent scales
Centre of lesion - atrophic ,scarred & depigmented with telangiectasia
Active border - is elevated, shows lilac erythema & hyperpigmentation
Persist for years & tend to expand slowly
Etiology?
Aggravating factor?
Etio → unknown
Aggravates on → UV exposure →.: photo protection is imp.
Explain carpet tack sign?
When scale is removed → its undersurface shows keratotic spikes → that have occupied dilated pilosebaceous canals
Dx?
Direct immunofluorescene?
Biopsy
Linear deposits of ig G,M,A & C3 at dermoepidermal junction
Histopath?
Hyper keratosis
Epidermal atrophy with follicular plugs
Basal cell degeneration
Perivascular & periappendageal lymphocytic infiltrate
Treatment?
Local lesion → topical or intralesional steroids
Resistant or disseminated lesion → hydroxychloroquine
