what type of murmur is this?

  • Almost always pathological
  • Early decrescendo murmurs signify regurgitant flow through an incompetent semilunar valve (usually aortic)
  • Rumbling diastolic murmurs in mid or late diastole point to stenosis of an AV valve (usually mitral)
  • Less common than systolic murmurs and harder to hear

o Location: 2nd to 4th left Intercostal spaces
o Radiation: if loud, to the apex; sometimes right sternal border
o Intensity: 1-3, best heard when patient sitting, leaning forward with breath held after exhalation
o Ejection sound present
o S3 and S4 if severe
o Arterial pulses bounding

o Location: usually limited to apex
o Radiation: little to none
o Intensity: 1-4; heard with bell exactly over the apical impulse with patient in left
lateral decubitus position
o S1 accentuated
o Opening snap and P2 may be accentuated with pulmonary hypertension
