Eye Movement Disorder in Schizophrenia
Abnormal smooth pursuit and saccades
Highest Psychiatric Disorder Category Prevalence (3)
- Anxiety
- Depression
- Substance Use
Pathological Gambling Psychiatric Comorbidity
Mood disorders, particularly MDD
Misinterpretation of Bodily Symptoms Dx
Illness anxiety d/o
Prolactin w/r/t Seizures
Often elevated in epileptic seizure
Internuclear Opthalmoplegia (disease, lesion, physical exam)
MS (but can also be other brainstem lesions)
MLF of brainstem
Impaired adduction of ipsilateral eye, coarse nystagmus of contralateral eye
Disinhibited Social Engagement Disorder
Child actively approaches and interacts w/ unfamiliar adults
Often history of unstable/insufficient care, preventing form of stable attachments
Reactive Attachment Disorder
Inhibited, emotionally withdrawn behavior and minimal social/emotional responsiveness
Also often history of insufficient upbringing
Inhalants Long-Term Risk
Very early onset dementia
Optic Neuritis (what it is and ds suggestion)
Eye pain that increases w/ movement, followed by scotoma in affected eye
Often initial presenting sx of MS
Marcus Gunn Pupil
Swinging flashlight test for ON: afferent pupillary defect
Pupil paradoxically dilates when light moved away from normal eye to affected eye
Uhthoff’s Phenomenon
Heat sensitivity in MS: incr core body temperature worsens sx
Lhermitte’s Sign (what it is and associations)
Transient electric shock down spine or extremities upon neck flexion
Most commonly associated w/ MS, can also be other injuries of cervical spinal cord
