- External Ear.
- Taste from Anterior 2/3 Tongue.
- Hard and Soft Palate.
Cranial Fossa and Facial Nerve.
Internal Acoustic Meatus.
Clinical Features of Intracranial Facial Nerve Palsy (3).
- Chorda Tympani - Reduced Salivation and Loss of Taste.
- Nerve to Stapedius - Sound Hypersensitivity.
- Greater Petrosal Nerve - Reduced Lacrimal Fluid Production.
Clinical Features of Extracranial Facial Nerve Palsy.
MOTOR - Paralysis or Severe Weakness of Muscles of Facial Expression.
Aetiology of Intracranial Facial Nerve Palsy.
- Infection related to external or middle ear.
2. Idiopathic - BELL’S PALSY.
Aetiology of Extracranial Facial Nerve Palsy (4).
- Parotid Gland Pathology.
- Infection - Herpes.
- Compression - Forceps Delivery.
- Idiopathic - BELL’S PALSY.
Clinical Features of Bell’s palsy (4).
- Acute Onset of Unilateral LMN Weakness (not Extraocular).
- Mild-Moderate Postauriculaar Otalgia.
- Hyperacusis.
- Nervus Intermedius (Altered Taste and Dry Eyes).
Management of Bell’s palsy.
- Prompt administration of oral steroids - Prednisolone 50mg for 10 days followed by a taper.
- Supportive- Artificial Tears and Ocular Lubricants.
- Refer urgently to ENT if no improvement after 3 weeks.
- Should recover within 3-4 months.
Features Suggestive of Ramsay-Hunt Syndrome (2).
- Prominent Otalgia.
2. Vesicular Rash in External Auditory Meatus, Palate or Tongue.

- 20-40.
2. Commoner in pregnancy.

Reactivation of VZV in Geniculate Ganglion in CN VII.
Management of Ramsay-Hunt Syndrome.
Oral Aciclovir and Corticosteroids.
Aetiology of Bilateral Facial Nerve Palsy (5).
- Sarcoidosis.
- GBS.
- Lyme disease.
- Bilateral Acoustic Neuromas (Neurofibromatosis Type II).
- Bell’s palsy.
Aetiology of Unilateral Facial Nerve Palsy - LMN (7).
- Bell’s palsy.
- Ramsay-Hunt Syndrome (Herpes Zoster).
- Acoustic Neuroma.
- Parotid Tumours.
- HIV.
- MS (can cause UMN too).
- Diabetes Mellitus.
