Anatomical Course of the Trochlear Nerve (5).
A
- Trochlear Nucleus - posterior aspect of Midbrain.
- Anterior and Inferior within SAH space.
- Pierce dura mater adjacent to Posterior Clinoid Process of Sphenoid Bone.
- Runs along lateral wall of Cavernous Sinus (with III, IV, VII and ICA).
- Enters Orbit via Superior Orbital Fissure.

ONLY Superior Oblique - contralateral muscle : depress and introit the eyeball.

Clinical Features of Palsy of Trochlear Nerve (4).
A
- Vertical Diplopia - Defective Downward Gaze.
- Torsional Diplopia (Subjective Tilting).
- Contralateral Head Tilt - Compensatory.
- Look Straight : Affected Eye Deviates Upwards and Rotates Outwards.
