increae in SOB and or cough, sputum that worsens in <1 days- can be accompanied by tachypnea, tachycardiac
What are 3 common causes of AECOPD
Pneumonia
PE
HF
usually viral resp infections
What is a mild vs moderate vs severe exacerbation
mild- treated with SABDs only
moderate- SABDS+ oral steroids +/- abx
severe- ER/hospital visit
What is the inital treatment of a AECOPD
SABA +/- SAMAs
When are oral steroids used in AECOPD
Severe exacerbations
What oral steroid and dose is used typically in AECOPD
40mg pred x 5 days
How long does a AECOPD ususally last
7-10 days
What vit deficiency should be looked at for patients with frequent exacerbations
Vit D - replace if necessary
When should abx be used in AECOPD
if bacterial cause
What are the 3 cardinal s/s it is a bacterial AECOPD
increased purulent sputum
increased sputum volume
increased SOB
What is first line abx tx and dosage for AECOPD
amox 1g TID 5-7 days
or doxycycline 200mg once then 100BID
or sulfa-trimeth 1DS BID
all are first line
