What is the first line treatment for chronic pain
non-opioids
NSAIDs, duloxetine, benzos
and PT, accupuncture, massage, exercise
When should opioid therapy be initiated for chronic pain
failed trial of non-opioid therapy
Examples of inital trial opioids
morphine, oxycodone, hydromorphone
codeine
tramadol
What should not be used for first line opioids
methadone
fentanyl
Who should avoid morphine
pts with renal insufficiency
What is a reasonable trail of opioid therapy for chronic pain
3-6 months
When does tolerance/withdrawl symptoms develop on opioids
2-4 weeks
Who should not use opioids for chronic pain
people with a history of/or active substance use disorder
What should an inital opioid dose be restricted to
<50mg morphine equvialents per day
For patients on opioids and not seeing relief what should be done
rotate to different opioid
First line treatment for neuropathic pain
antiepileptic meds like pregablin/gabapentin
and antidepressant meds like amitriptyline, duloxetine
Question about whether to start narcotics for ankle pain
