COPD - Acute/Infective Exacerbation Flashcards
1
Q
Causative Organisms of COPD IE (4).
A
- H. influenze (commonest).
- S. pneumoniae.
- M. catarrhalis.
- Viruses (30%) - RSV.
2
Q
Medical Management at Home (3).
A
- Prednisolone 30mg once daily for 7-14 Days.
- Regular Treatment.
- Antibiotic - if bacterial infection.
3
Q
Medical Management at Hospital (4).
A
- Steroids e.g. 200mg Hydrocortisone or 30-40mg Oral Prednisolone.
- Nebulised Bronchodilators.
- Oral Antibiotics - Infection (AMD) if Purulent.
- Physiotherapy - Sputum.
4
Q
Advanced Management at Hospital (4).
A
- IV Aminophylline.
- NIV = BiPAP.
- Intubation and Ventilation.
- Doxapram (Respiratory Stimulant).
5
Q
Why should too much Oxygen detrimental to an IE of COPD patient? (2)
A
- Depression of Respiratory Drive.
- Slowed Breathing Rate and Effort - CO2 retention.
6
Q
How is Oxygen Therapy given in COPD?
A
Venturi masks - deliver a specific percentage concentration by leaking out side of the mask and normal air to be inhaled too.
7
Q
Types of Venturi Masks (6).
A
- Blue - 24%.
- White - 28%.
- Orange - 31%.
- Yellow - 35%.
- Red - 40%.
- Green - 60%.
8
Q
What are the targets in COPD Oxygen Therapy?
A
- If CO2 retaining, aim for Oxygen saturations of 88-92% by Venturi Mask.
- If not CO2 Retaining, Oxygen for SpO2 > 94%.
9
Q
How is a CO2 Retainer identified?
A
Bicarbonate is elevated - compensation.
