Colonic polyps
Masses protruding into gut lumen Sawtooth appearance 90% non neoplastic Often rectosigmoid Can be tubular or villous
Adenomatous
Adenomatous are precancerous
Malignant risk increased with size, villous histology, epithelial dysplasia
Precursor to colorectal cancer (CRC)
The more villious the more polyps, the more likely malignant (villous=villainous)
Polyp symptoms: often asymptomatic, lower GI bleed, partial obstruction, secretory diarrhea.
Hyperplastic
Most common non neoplastic poly in colon
>50% in rectosigmoid colon
Juvenile
Mostly sporadic lesions in children <5 yrs
80% in rectum
If single, no malignant potential
Juevenile polyposis syndrome: multiple in GI
increased risk of adenocarcinoma
Peutz-Jeghers
Single polyps are not malignant
Peutz-Jeghers: AD multiple nonmalignant hamartomas throughout GI, hyperpigmented mouth, lips, hand, genitalia
Associated with increased risk of CRC an other malignancies.
