Theophylline MoA
A
Blocks phosphodiesterase → increases cAMP levels → epinephrine release from adrenal medulla cells → bronchodilation, CNS and cardiac stimulation, diuresis, gastric acid secretion

When is theophylline used in asthma treatment?
A
MAINTENANCE, but not first line!!

Theophylline ADEs
A
Nausea, loose stools, HA, tachycardia, insomnia, tremor, nervousness

Theophylline toxicity ADEs
A
Persistent, repetitive vomiting, ventricular tachycardias, seizures

Why isn’t theophylline not used often anymore?
A
Narrow therapeutic index and high risk of ADEs
