Chapter 19 Flashcards
Crew resource management CRM
is a behavioral approach to reducing human error in high risk or high consequence activities.
Dupont’s “ dirty dozen” are comprehensive list of reasons and ways that humans make mistakes
- Lack of communication
- Complacency
- Lack of knowledge
- Distraction
- Lack of teamwork
- Fatigue
- Lack of resources
- Pressure
- Lack of assertiveness
- stress
- Lack of awareness
- Norms
Active Failures
are the unsafe acts committed by people who are in direct contact with the situation or system
Latent conditions
are the inevitable “resident pathogens” within the system,
Inquiry is the
process of questioning a situation that causes concern
Advocacy is
the statement of opinion that recommends what the person believes is the proper course of action under a specific set of circumstances
CRM promotes
members working together for the common good
Knowledge is
often closely associated with power
Human error is
a normal inevitable occurrence
Recognition-primed decision making RPD
describes how commanders can recognize a plausible plan of action
Naturalistic decision making
describes how commanders make decisions in their natural environment
Situational awarenss
is the accurate perception of what is going on around you
Six steps should be followed to maintain emergency scene situational awareness
- Fight the fire
- Assess problems in the time available
- Gather information from all sources
- Choose the best option
- Monitor results and alter the plan as necessary
Ambiguity
An event, order, or message that has more than one potential meaning
Distraction
Anything that takes attention away from the larger mission. Distraction can come in the form of outside influences or inside influences ( cultural and personal biases)
Fixation
tunnel vision
Overload
More things are happening than ope person can process
Complacency
When humans perform the same process repeatedly, they tend to become bored due to the hypnotic effect of repetitive action. Inattentiveness breeds carelessness.
Improper procedure
Immediate action is required to correct these mistakes
Unresolved discrepancy
Indication and observations that are in conflict with expectations must not be ignored. Fire fighters must be trained to recognize discrepancies as needing attention immediately
Nobody fighting the fire
An IC may find his or her attention drawn to actions of one specific company, causing the leader to ignore the rest of the incident.
