Psoriasis
Alternating neutrophils and parakeratosis in stratum corneum (sandwich sign), neutrophilic spongiform pustules, tortuous blood vessels in dermal papillae.
What lesions can present with collections of neutrophils within the stratum corneum?

Plaque psoriasis
Sandwich sign, neutrophilic spongiform pustules, regular bulbous acanthosis and thin superpapillary plates.
Pustular psoriasis
Neutrophils in the stratum corneum with subcorneal and spongiform pustules.
Guttate psoriasis
Neutrophils on top of parakeratosis. No acanthosis.
ILVEN
Alternating orthokeratosis (with granular layer) and parakeratosis (without underlying granular layer)
Mycosis fungoides
Epidermal collection of hyperchromatic lymphocytes surrounded by white space. Spongiosis and papillary dermal fibrosis.
Syphilis
Vacuolar interface dermatitis with elongated psoriasiform acanthosis. Neutrophils in stratum corneum. Perivascular lymphoplasmacytic infiltrate. Endothelial swelling.

Compact hyperkeratosis with parakeratosis and preserved granular layer.
Porokeratosis
Cornoid lamellae with intervening lichenoid or psoriasiform zones
Acute spongiotic dermatitis
Spongiosis, exocytosis of lymphocytes
Seborrheic dermatitis
Psoriasiform spongiotic dermatitis with neutrophilic scale/crust at edges of follicular ostium
Subacute spongiotic dermatitis
Parakeratosis, acanthosis, spongiosis, and exocytosis of lymphocytes
