Synthetic osteochondral resurfacing (SOR) for treatment of large caudocentral OCD lesions of the humeral head in 24 dogs

1st generation (G1) and 2nd generation (G2) SOR implants reviewed
Osteochondral resurfacing with G1 in 13 dogs (15 shoulders) and G2 11 dogs (13 shoulders)
Lameness resolved in all dogs tx with G1 implants and in 10/11 dogs tx with G2 implants by 12 wks after surgery
1 dog with G2 implant had implant-associated infection needing removal
No recurrence of lameness in 9/15 limbs with G1 implant and 10/11 limbs with G2 implants (median 387 and 365 d post-op)
Mean postop LOAD scores 4.2 of 52 and 5.2 of 52 tx with G1 vs G2
Good clinical results overall
Synthetic resurfacing was technically feasible in all joints of this study and resulted in good clinical results.

COR system
The COR system has a cutting tooth on a harvester blade, which differs from the previously described method of obtaining grafts.
the T-handle guide was removed, a specific no-impact transfer case (graft loader) was used to push the graft in the delivery guide.
The cutter part of the COR system was then removed from the guide by twisting the bayonet ring to the unlock position.
The harvester/delivery guide protected the graft and allowed inspection of the quality, length and shape of the graft.
The graft remained within the harvester/delivery guide until it was ready for insertion into the OCD lesion.
Appropriately-sized sockets were created in the recipient site with the calibrated drill to allow insertion of the donor grafts using the delivery guide.
The grafts were seated with the dedicated tap, the guide was removed, and if required, a universal tamp was used to adjust the final placement of the graft
Single graft transfer in 6 and mosaic in 14
No major and three minor postop complications
3 months post-op 15/20 (75%) had no lameness or radiographic signs of inflammation
6 months 12/13 dogs had no lameness
Conc: Treatment of OCD of the femoral condyle using the COR system is feasible in dogs, with low complications and provides a satisfactory short-term outcome.
