define cardiomyopathy
this is any disease of the heart muscle
four types of cardiomyopathy
- dilated
- hypertrophic
- restrictive
- Arrhythmogenic right ventricular dysplasia
describe dilated cardiomyopathy
this is a dilated heart which leads to a low ejection fraction presenting with SOB and poor exercise tolerance
define hypertrophic cardiomyopathy
this is increased muscle in the myocardium leading to strong contraction with diastolic dysfunction
management of hypertrophic cardiomyopathy
- beta blockers and verapamil for symptoms to reduce force of contraction
define restrictive cardiomyopathy
the heart muscle becomes rigid and less elastic due to deposited in the myocardium
define Arrhythmogenic right ventricular dysplasia
the right ventricle becomes replaced by fat
what does Arrhythmogenic right ventricular dysplasia present with?
- syncope
- arrhythmia (AF)
- sudden death
what does restrictive cardiomyopathy have to be distinguished from?
constrictive pericarditis
what is constrictive pericarditis
the pericardium becomes rigid
diagnosis of constrictive pericarditis
signs of right sided heart failure (rising JVP, Kussmauls sign, etc.)
how to distinguish between restrictive cardiomyopathy and constrictive pericarditis
CT
MRI
diagnosis of cardiomyopathy
- CXR
- echocardiogram
- ECG (LVH)
- stress testing
- cardiac catheterisation
management of cardiomyopathies
- ICD (if arrhythmias present)
- VAD
- pacemaker and radio frequency ablation
- transplant
