chest pain or discomfort brought on by activity or emotion due to myocardial iscahemia, relieved by rest
presentation of stable angina
- pressure on chest, can radiate
- relieved by rest
diagnosis of stable angina
- exercise testing
- ECG
- perfusion scanning
- CT angiogram (50-70% narrowing is considered an obstruction)
management of stable angina
step one: beta blocker or CCB (GTN as an adjuvant for acute episodes)
step two: consider mono therapy e.g. ISMN, ivabradine, nicorandil or ranolazine
define unstable angina
this is angina that can occur at rest
presentation of unstable angina
- exacerbation of current angina
- chest pain not relieved by rest
diagnosis of unstable angina
- relieved by GTN
- ECG to rule out MI
- cardiac biomarkers are not elevated
- GRACE risk calculator for ACS
management of unstable angina
- relieved by GTN
- beta blocker or CCB
- stain, anticoagulation
