Biopsies Flashcards
Types of Ultrasound Guidance
Liver
Thorocentesis
paracentesis
breast core
breast cyst aspiration
breast needle localizations
fine need thyroid aspirations
hip aspirations
PICC lines
perma cath insertions
Liver Biopsie
any type of treatment for hepatitis to know what form of treatment to follow
done as outpatient
patient stays for 2 hrs post
radiology nurse monitors vitals
LIver biopsy procedure
patient NPO
informed consent
full assessment by nursing
labs prior PT,PTT,INR
supine position
sonographer locates and marks position, the depth is measured and image is saved for documentation
procedure Time Out
everyone present verification of paitne ID and all equipment and supplies are present and patient agrees
Thorocentesis
done for pleural effusions when patient becomes SOB or fluid needs testing for diagnoses
done on one side per day due to chance of pneumothorax development
Common Thorocentesis reasons
cancer
post op CABG
liver failure
congestive heart failure
Thorocentesis postion
sitting with legs over edge of cart
arms placed foward on bedside table
Paracentesis
doen for ascites to remove peritnial fluid
can be large volume up to 15 liters
offers great patient relief
therapeutic or diagnostic
paracentesis reasons
cancer
liver failure (cirrhosis
CHF
Breast core biopsy
outpatient procedure
check for solid mass (benign/malignant)
type of cancer
Breast cyst aspiration
done for patient relief or to make sure fluid is able to be aspirated and not solid mass
no fluid proceed to biopsy
Breast needle localization
done for patients going inot surgery
wire left in place for surgeon to locate mass
Thyroid fine needle aspiration
performed if solid nodule over 1.5cm
most nodules are benign
25g needle usually 5 passes
cytology present to make slides for pathologist to ensure enough smaples are obtained
Thyroid needle aspiration position
patien is supine
pillow under shoulders to extend neck
PICC line
peripherally inserted central catheter
PICC line reasons
can be used for long periods of time
chemotherapy regiments
extended antibiotic therapy
parenteral nutrition
PICC procdures
inserted in peripheral vein
cephalic
basilic
brachial
then advanced to larger toward heart
tips rests in the distal superior vena cava
PICC line procedures
sterile suite
sonographer must glove and gown
machine wiped clean and covered
vein located and needle inserted under ultrasound guidance
Perma Catheter
employed in dialysis patients who require ongoing nutrition via veins
placed in internal jugular
same procedure as PICC line
Sterile
free of bugs that can infect
applies to select surfaces, objects or substances that will be introduced to patients body
some opbjects can’t be
Sterile technique
first and foremost about minimizing possible sources of infection
nosocomial infection
infection got while in hospital
Sterile field
sterile surfaces
scrubbed surgical personnel
hands washed iow OR protocol
donning sterile gown/gloves
sterile surface extends from chest to waist
elbows to tips of gloved fingers
rest is not sterile
