Anatomy of Surgical incisions Flashcards
Describe the external obliques?
hands in pocket
attatch between the lower ribs + iliac crest, pubic tubercle and linea alba
Describe the internal obliques?
attach between the lower ribs, thoracolumbar fascia, iliac breast and linea alba
Describe the transversus abdominus?
attach between the lower ribs, thoracolumbar fascia, iliac crests and linea alba
What divides the rectus abdominus?
tendinous intersections
Where does the linea alba run between?
xiphoid process -> pubic symphysis
What is the rectus sheath?
combined aponeuroses of the anterolateral abdominal wall muscles
What is the only thing cut in a lower segment cesarean section?
rectus sheath
What is the nerve supply to the anterolateral wall muscles?
7th-11th intercostal nerves
subcostal T12
iliohypogastric - L1
ilioinguinal - L1
What is the blood supply to the anterior anterolateral wall?
superior and inferior epigastric arteries
they lie posterior to the rectus abdominus
What is the blood supply to the lateral anterolateral wall?
intercostal and subcostal arteries
What is the procedure of a lower segment cesarean section (LSCS)?
rectus sheath is cut then the rectus muscles are separated from each other in a lateral direction, towards their blood supply
What layers need to be closed in a LSCS?
uterine wall with visceral peritoneum
rectus sheath
fascia if high BMI
skin
What layers are opened in a laparotomy?
skin and fascia
linea alba
peritoneum
What layers need to be closed in a laparotomy?
peritoneum and linea alba
fascia if increased BMI
skin
What type of iscision is a laparotomy?
midline incision - relatively bloodless meaning poor healing and increased chance of complications
