AMICAR MOA
Inhibition of plasminogen activators to produce fibrinolysis-inhibitory effects
AMICAR Indications
Enhances hemostasis when fibrinolysis contributes to bleeding post cardiac procedures
AMICAR Contraindications
Active intravascular clotting process
Concern of DIC or primary fibrinolysis
Factor IX Complex concentrates - risk of thrombosis
Difference between primary fibrinolysis and DIC
Platelet count decreased in DIC, not in primary fibrinolysis
Protamine paracoagulation test positive in DIC, negative in primary fibrinolysis
Euglobin clot lysis test abnormal in primary fibrinolysis, normal in DIC
AMICAR Risk Factors
Upper urinary tract bleeding
Possible damage to muscle tissue with excessive doses, causes increased CKD levels
AMICAR Dosing
5g bolus, followed by 1.25g/hr for 8 hours or until bleeding stopped
Excessive dosing is counterintuitive, 20g is a recommended limit but with no research support
