Primary amenorrhoea
absence of menstruation by 16 years of age
Secondary amenorrhoea
absence of menstruation for more than 6 months in a normal female of reproductive age which is not due to;
- Pregnancy, lactation or menopause
Anatomical causes of amenorrhea
- Genital tract abnormality
- Asherman’s syndrome - intra uterine adhesions preventing menstruation
- Mullerian agenesis - congenital malformation where mullerian ducts fail to develop
Ovarian causes of amenorrhea
- PCOS - annovulation
2 Premature ovarian failure
Premature ovarian failure
defined as cessation of periods before 40
Pituitary disorders
- Adenomas of the pituitary gland - prolactinomas are the most common
- Pituitary necrosis - Sheehan’s syndrome
Other conditions causing amenorrhea
- Kallman’s syndrome
2. Systemic disorders e.g. sarcoidosis, tuberculosis
Ix of amenorrhoea
- Pregnancy test
2. Bloods for LH, FSH, testosterone, prolactin testing
Results show raised LH or testosterone, this suggests cause is…
Polycystic ovarian syndrome
Results show raised FSH
Suggestive of Premature ovarian failure
Results show high prolactin
Prolactinoma
Ultrasound scan
Check for poly cysts on the ovaries
When may hysteroscopy be indicated?
Where Asherman’s or Cervical stenosis is indicated
Tx for Hyperprolactinaemia due to prolactinoma
Dopamine agonist - bromocriptine
Tx for Asherman’s
- Adhesiolysis and IUD insertion
