What is acute interstitial nephritis?
A
Inflammation of the renal tubulo-interstitium

What causes AIN?
A
- Drugs mainly penicillin, rifampicin, NSAIDs, allopurinol and furosemide
- SLE/Sarocidosis/Sjogrens
- Some infections e.g. Staphy
Autoimmune:SLE/sjrogens/sarcoidosis
Infection: staph infection
NSAIDs, rifampicin, penicillin

What is the pathophysiology of AIN?
A
Histology shows marked interstitial oedema and interstitial infiltrate in the connective tissue between renal tubules

How does AIN present?
A
- fever, rash, arthralgia
- eosinophilia
- mild renal impairment
- hypertension

How is AIN investigated?
A
Urinalysis shows sterile pyuria and white cell casts

How do you differentiate between ATN and AIN?
A
AIN is an inflammatory process so there is a higher white cell content in the urine, while ATN is not so the urine has no cellular component
