3 bacteria account for 80% of cases
Etiology of Acute Bacterial Meningitis
Bacteria reach meninges by blood, extension from nearby infected structures, and communication of CSF with exterior — Accumulation of exudate damages cranial nerves, induces thrombophiebitis leading to ischemia —- Damaged cell membranes disrupt blood-brain barrier causing brain edema —- Intracranial pressure rise, BP drops (septic shock) and death can occur
Pathophysiology of Acute Bacterial Meningitis
Unexplained fever in infants 3mo-2y warrants lumbar puncture, Culture CSF sample
Diagnosis of Acute Bacterial Meningitis
Abrupt neck flexion with patient in supine position results in involuntary flexion of hips and knees
Brudzinski’s sign
Antibiotics, supportive for fever and dehydraion
