You are called by the partner of a 25-year-old man. They are concerned that since his return from Nepal 12 days ago he has had a high fever and one episode of watery diarrhoea that lasted 24 hours and subsided without treatment. Today he has started to feel feverish with a dull frontal headache and anorexia. His temperature rose to 39.4°C (103°F) and his headache and general fatigue worsened. He has also developed a dry cough and abdominal pain.
