What is the main pathophysiology in amniotic fluid embolisation?
amniotic fluid contains fetal tissue and triggers and IMMUNE REACTION in mum. (More like anaphylaxis than a VTE).
What is the mortality from amniotic fluid embolisation?
about 20%
What are the main risk factors for amniotic fluid embolisation?
- Increasing maternal age
- Induction of labour
- Caesarean section
- Multiple pregnancy
How does amniotic fluid embolisation present?
Around labour, similar to sepsis / PE / anaphylaxis, act onset of:
- Shortness of breath
- Hypoxia
- Hypotension
- Coagulopathy
- Haemorrhage
- Tachycardia
- Confusion
- Seizures
- Cardiac arrest
What is the management of amniotic fluid embolisation?
Supportive, no specific treatments. Initial ABCDE
Emergency, get help: experienced obstetricians, medics, anaesthetics, intensive care teams and haematologists
Transfer to ICU
