What are the types of breech?
Complete
(Incomplete)
Extended / Frank
Footling
What happens in complete breech?
legs are fully flexed at the hips and knees
What happens in incomplete breech?
one leg flexed at the hip and extended at the knee
What happens in extended / frank breech?
both legs flexed at the hip and extended at the knee
What happens in footling breech?
foot is presenting through the cervix with the leg extended
What can be done to correct breech? When?
ECV - external cephalic version
use at 36 weeks for nullips and 37 for previous mums
What options are offered for delivery of breech?
vaginal - safer for mum (need experienced team and theatre nearby for 40%)
ELCS - safer for fetus (and if first twin is breech)
What two things are given to mum for an ECV?
- SC terbutaline tocolysis (beta-agonist)
- antiD if she is Rh-ve (plus Kleihauer test)
