(9) Formation of Bilirubin Flashcards
Components of protein
CHON
o Type of bilirubin formed / synthesized in the circulation.
o Bilirubin bound to albumin
B1/unconjugated:
Bilirubin is a waste product, therefore, should be excreted.
o Bilirubin can only be excreted if it is converted into its conjugated form (B2).
o B2 is not normally found in circulation but very small amount (to none) of B2 leaks in the blood
Enzyme responsible for conjugation of bilirubin
Uridine Diphosphate Glucoronyl / Glucoronosyl transferase (UDPGT):
orange-colored product which gives stool its brown color.
Stercobilin:
principal product that gives urine yellow color.
Urobilin
In the past, icterus & jaundice are used interchangeably.
Fetus don’t develop jaundice; they develop it after birth.
B1 can cross the blood-brain barrier (BBB) so it is toxic
→ leads to kernicterus
Kernicterus:
o Bilirubin deposition in the brain.
o Causes severe motor dysfxn & retardation.
Bilirubin most especially B1 are photosensitive.

Prehepatic jaundice disorders:
o HA
o G6PD def.
Prehepatic jaundice disorders:
o HA
o G6PD def.
Hepatic Unconjugated hyperbilirubinemia disorders:
o Gilbert’s
o Crigler-Najjar
o Lucey-Driscoll
- Conjugated hyperbilirubinemia disorders:
o Dubin-Johnson
o Rotor syndrome
can ONLY measure Direct/B2/Conjugated bilirubin & never B1.
