How is molluscum contagiosum transmitted?

A

Transmitted by direct skin contact but fomites (bath towels or sponges) or swimming pools can also be a source of infection. Can also be sexually transmitted and is associated with genital molluscum lesions.

how to treat molluscum contagiousum?

A
  • Picking out the soft white core - could lead to autoinoculation
  • Cryotherapy - can leave white marks → useful in patient with only a few small lesions
  • Gentle Curettage - can scar → → useful in patient with only a few small lesions
    • children may need local anaesthetic → lidocaine + prilocaine
  • Laser Ablation - can scar
  • as per eTG → an occlusive tape can be left on lesions for 24 hours and then pulled off rapidly!!

  • reassurance

- urea 10% topically if needed

Treatment for paronychia?

A
  • consider ring block → 1-2ml lidocaine (no adrenaline) into proxmial crease of finger
  • Incise the abscess with a 21G needle → immediate relief
  • No need for ABx unless surrounding cellulitis
    • dicloxacillin or flucloxacillin 500mg PO Q6H for 5/7
    • cefalexin 1g BD for 5/7

Management of Herptic Gingiostomatitis?

A
  • hydration
  • topical anaesthetic - benzydamine 1% gel, topically to lesions, 2-3 hourly as necessary
    • can up to lidocaine 2% viscous solution
  • DO NOT NEED antiviral treatment usually
    • only in severe or immunocompromised - use antivirals

gluten free diet as it is always related to gluten sensitive enteropathy.
