44: Pediatric Renal Flashcards
UTI sx in children
just like in adults - fever, nausea, foul smelling urine, suprapubic pain, frequency, urgency, dysuria, incontinence
two best ways to get a urine specimen in a child still in diapers
- cath the bladder
2. suprapubic aspiration
suprapubic aspiration
needle inserted into bladder thru suprapubic abdomen
bag urine samples cons
- only helpful if negative
2. not appropriate for culture
UTI Dx criteria in a clean catch sample vs catheter vs suprapubic aspiration
- clean catch: pyuria + 50,000+ CFU
- cath: pyuria + 50,000+ CFU
- suprapubic aspiration: pyuria + any amount of growing culture (low risk of contamination)
MC pathogen for UTI in kids
e. coli
two MC causes of obstructive uropathy in kids
- posterior urethral valves (boys only)
2. ureteropelvic junction obstruction
significance of VUR (vesico-ureteral reflux) grading
higher grade -> increased risk of UTI, renal damage/scarring
treatment for VUR
most resolve on its own, but grades 4 and 5 require surgical repair
best prognostic indicator for long-term renal damage in HSP
amount of protein in the urine
empiric Abx treatment for UTI in kids
must have coverage for E coli
indications to refer a peds pt to a specialist bc of a UTI
- vesico-ureteral reflux grades 3-5
- obstructive uropathy
- renal anomalies
- compromised renal function
- HTN
- bowel and bladder dysfunction
