three main Abx for UTI
TMP-SMX, fosfomycin, nitrofurantoin
normal flora in the urethral region
lactobacilli, coagulase-negative staph, corynebacterium, strep
asymptomatic bacteriuria
organisms in urine but not causing illness or symptoms
uncomplicated UTI
- uncomplicated: in a healthy, non-pregnant, pre-menopausal female with normal anatomy
recurrent UTI
2+ in 6 months or 3+ in 12 months
UTI RFs
outflow obstruction, inadequate fluid intake, voiding dysfx, sex, spermicide, estrogen depletion, catheters, incontinence
symptoms of acute complicated UTI
simple cystitis sx + fever, chills, fatigue, flank pain, pelvic pain
RFs for MDR gram negative UTI
- urine culture positive for MDR gram (-)
- inpatient stay at healthcare facility
- use of fluoroquinolone, TMP-SMX, or broad spectrum beta-lactam
- travel to india, israel, spain, or mexico
all pts with indwelling catheter develop what?
bacteriuria - most wont develop UTI tho, even if urine has a cloudy look with foul odor
