Safety
- Avoiding injuries to patients from the care intended to help them
Effective
- Providing services based on scientific knowledge to all who can benefit and refraining from providing services to those who would most likely not benefit
Patient Centered
- Care should be respectful and responsive to individual patient preferences, needs and values. Patient values should guide all clinical decisions
Timely
- Reducing waits and sometimes harmful delays to care
Efficient
- Avoid waste, including equipment, supplies, ideas, and energy.
Equitable
- Care should not vary in quality due to gender, ethnicity, geographic location, and socioeconomic status
National Goals/Strategies for Quality Improvement
National Healthcare Quality and Disparities Report
- Provides a comprehensive report of quality healthcare received by the general US population and disparities in care.
6 Priorities
- Patient safety, patient centered care, care coordination, effective treatment, healthy living, care affordability.
Centers for Medicare/Medicaid Services (CMS)
- Service that allows consumers to compare information on hospitals and other care settings.

CMS Core Measures
- Established set of standard national measures which are reevaluated every year.
National Quality Forum
- Non-governmental organization to measure quality care
National Database for Nursing Quality Indicators
- National nursing database that provides quarterly and annual reporting of structure, process, and outcome indicators to evaluate nursing care at the unit level

- National committee for quality assurance
- Accreditation association for ambulatory health care
- American nurses association magnet recognition
- The joint commission
Benchmarking
- Process of comparing practice performance with external standards
- Organization compares their own processes with other similar organizations to measure their own quality
- Essential to provide safe, quality, efficient care
Performance Measure
- Specific measurements to gauge performance on clinical processes and outcomes.
- Serves as an indicator on progress towards a benchmark or goal
Commonly reported as rates
- How many times has the measurement been met
- How many opportunities were there to meet the measure
Standard Measurement
- Helps identify when performance is below and agreed upon standard and signals the need for improvement.
- Benchmarking can help share best practices when we recognize performance is above recommended benchmarks
Quality Indicator
- Standardized, EBP measure of healthcare quality that can be used to track clinical performance and outcomes
Dashboard
- Reporting tool that provides interfaces to track and monitor healthcare metrics
Nursing Sensitive Indicator
- Measures that reflect structure, process, and outcome of nursing care.
