A
  • Gather complete assessment data
  • Develop skeleton diagram of patients health problems
  • Analyze and categorize specific data
  • Indicate relationships between medical and nursing diagnosis
  • Develop patient goals, outcomes, nursing interventions for each diagnosis
  • Evaluate actual patient response to each intervention and summarize clinical impressions

Steps of a Concept Map

A

Step 1 - Place patient’s medical diagnosis at the center
Step 2 - Place major problems due to diagnosis based on assessment data in surrounding boxes
Step 3 - Analyze and categorize the data, find evidence for problems in the surrounding the boxes
Step 4 - Same as step 3
Step 5 - Draw lines connecting medical diagnosis with identified problems
Step 6 - Change outside problems into official nursing diagnosis
Step 7 - Prioritize each nursing diagnosis by ranking them in importance (numbering them)
Step 8 - Draw lines between nursing diagnosis that relate to one another
Step 9 - Insert interventions next to the problems.
