What are the steps of the nursing process?
Assess Identify the problem Plan Implement Evaluate
Process of priority setting
High priority: greatest threat to the patient
Medium priority: non-threatening diagnoses
Low priority: diagnoses not specifically related to current health problem
Goal Statement Components:
Subject Action Performance criteria Target time Special condition
Goal:
Expected outcomes derive directly from nursing diagnosis
Problem statement describes the response/health status to be changed
Desired outcome states the opposite of the problem response.
Client Outcomes must be:
specific realistic measurable indicate a definite time frame for achievement consider clients desires and resources

Nurse initiated intervention:
actions performed by a nurse without a physican’s order. (independent)
physician initiated intervention:
actions initiated by a physician in response to a medical diagnosis but carried out by a nurse under doctor’s orders. (dependent)
