X - Hematologic Agents Flashcards
Iron Deficiency Anemia
Ferrous Sulfate (Fumarate - ↑ elemental Fe)
Megaloblastic anemia, vitamin B12 deficiency
Cyanocobalamin
Megaloblastic anemia, prevention of neural tube defects (spina bifida)
Folic Acid
Anemia in CKD
EPO
Neutropenia, agranulocytosis
Filgrastim
Chemotherapy-induced thrombocytopenia
Oprelvekin
Acute treatment of DVT, PE and AMI, PTT monitoring, SE: bleeding, thrombocytopenia
Heparin
Low molecular weight heparin, less risk for thrombocytopenia, does not need monitoring
Enoxaparin
Treatment of heparin-induced thrombocytopenia
Lepirudin
Antidote to heparin-induced bleeding
Protamine Sulfate
Chronic anti-coagulation, SE: bleeding, skin necrosis, many drug interactions
Warfarin
Antidote to warfarin (immediate reversal)
Fresh Frozen Plasma (FFP)
Antidote to warfarin (slow reversal)
Vitamin K
Test for intrinsic pathway function
PTT
Test for extrinsic pathway function
PT
Why is an overlap between heparin and warfarin done?
warfarin’s effect requires elimination of preformed clotting factors (8-60h), bypass initial prothrombotic effect of warfarin (skin necrosis)
Thrombolysis in AMI, ischemic CVD and PE, SE: bleeding
Alteplase (rtPA)
Bacteria-derived thrombolytic, decreased effect on subsequent uses due to antibody formation
Streptokinase
Antidote to thrombolytic overdose
Aminocaproic/Tranexamic Acid
Irreversible COX inhibitor, instant prevention of arterial thrombosis, SE: tinnitus, hypersensitivity, Reye syndrome, Samter triad (asthma, NSAID sensitivity, and nasal/ethmoidal polyposis)
Aspirin
ADP inhibitor, additive effects with aspirin
Clopidogrel
Glycoprotein IIb-IIIa inhibitor
Abciximab
Phosphodiesterase inhibitor, cardiac stress testing
Dipyridamole
Toxic Dose of Aspirin
150 mg/kg
Lethal Dose of Aspirin
500 mg/kg
Aspirin Hypersensitivity
Samter Triad (asthma, NSAID sensitivity, and nasal/ethmoidal polyposis)
Aspirin Toxicity
Coma, Hyperventilation, Acidosis (HAGMA), Fever, Seizure
Acid-base abnormality in salicylate poisoning
respiratory alkalosis with HAGMA
Aspirin Intoxication: Adults
mixed acid-base disorder (respiratory alkalosis with HAGMA)
Aspirin Intoxication: Children
pure acid-base disorder
Completely halt the ETC
ETC Inhibitors
Dissipate proton gradient without interrupting ETC
Uncouplers
