Factors affecting wound healing
Local - foreign body, infections, ischemia.
Systemic - age/gender, hormones, stress, diseases, obesity, alcoholism, nutrition, immunocompromised.

Stages of primary wound healing - 1st intention (regeneration)
Inflammation
Proliferative
Maturation
Primary healing (1st intention) - inflammation
First few hours wound becomes inflamed and blot clot forms.
Cell debris and platelets fill the space.
Cellular involvement: macrophages and fibroblasts.
Primary healing (1st intention) - proliferation
Epithelial cells proliferate across wound and begin closure process.
Replace damaged cells.
Clot becomes scab and separates after 3-10 days.
Primary healing (1st intention) - maturation
Stronger collagen laid down.
Excess blood vessels removed
Scar looses redness.
Stages of secondary wound healing - 2nd intention (repair)
Inflammation
Proliferative
Maturation
Secondary healing (2nd intention) - inflammation
Inflammatory reaction more intense.
Slough on top of wound - necrotic tissue.
Secondary healing (2nd intention) - proliferation
Blood supply is plentiful to reduce/prevent infection.
Granulation tissue forms at base of wound.
Secondary healing (2nd intention) - maturation
Collagen laid down horizontally.
Pronounced wound contraction.
Uneven scar tissue.
1st intention wound healing
Clean incision.
Limited scaring and wound contraction.
2nd intention wound healing
Ulcers or lacerations.
Often scaring and wound contraction.
Tertiary healing
Wounds left open for debridement.
Destructed tissue removed.
After period of time try suture walls together.
