what is wolf parkinson white
congenital pre-excitation syndrome that occurs due to the presence of an accessory electrical pathway between the atria and ventricles. It predisposes the patients to supraventricular tachycardias, especially AVRT
classification of WPW
type A = pathway between left side
Type B = pathway between right side
signs
none, palpitations, dizziness, syncope
investigations of WPW
ECG (delta, short PR, Broad QRS), bloods, imaging (echo, cardiac catheterisation)
long term management of WPW
anti-arrhythmics (amiodarone, sotalol), radiofrequency ablation
Contraindications in WPW
AVN blockers such as digoxin, adenosine and non- dihydropyridine calcium-channel blockers
emergency management of WPW
synchronised cardioversion, vagal manouevres, adenosine
complications of WPW
atrial fibrillation can pass to the ventricles via accessory pathway causing ventricular fibrillation and cardiac arrest
