Patho of Acne
1) hair becomes hyper-keritinized (thicker)
2) sebaceous glands produce extra sebum, and this along with dead skin cells become sticky and get stuck around the thicker hair follicle and form a plug
3) produces a comedone
2 types of comedones
1) Open (blackheads)
2) Closed (whiteheads)
3 Types of Acne
1) Mild - open and closed comedones
2) Mild to Moderate - papules, pustules, or both
3) Moderate to Severe - Nodules and cysts
1 causative agent of acne
P. Acnes (Propionibacterium acnes)
Treatment of acne
1st line- topicals (mono or combo therapy for mild)
** try a couple before moving to 2nd line
- topical benzoyl peroxide
- topical retinoids
- topical antibiotics
2nd line - oral antibiotics
3rd line - Accutane
Treatment of mild noninflammatory comedonal acne
- topical - benxoyl peroxide then topical retinoid
Treatment of moderate inflammatory papulopustular acne
- add topical antibiotic
Treatment of severe inflammatory acne
- oral antibiotic combined with topical combination therapy - topical antibiotic and/or retinoid
- if ineffective - start accutane
