Week 12: Beta Adrenergic antagonist (BB), Combined Alpha/Beta Agonist and Antagonist Flashcards
how do beta blockers work?
antagonists so block the receptors
slow SA node, slows HR, allows LV fill completely and lowers workload
metoprolol, atenolol are what kind of beta blockers? MOA and indication
selective bb
HF, HTN, afib, angina, post MI
block beta 1 receptors at SA node to decrease HR, decrease contractility in atria and ventricles, slow AV conduction
metoprolol, atenolol c/c
don’t stop abruptly!!! rebound HTN/tachyc
taper off 1-2 weeks
NO in pts with: bronchospasms, 2nd/3rd AV block
caution: diabetic, peripheral vascular dz, hyperlipidemia
NO in 1st trimester
metoprolol, atenolol ADE
Hypotension (orthostatic), bradycardia, dizziness, drowsiness, fatigue, insomnia, depression, pruritus, impotence, decreased libido, anaphylactic reactions (rare)
Impotence / decreased labido
Fatigue - goes away with time
Take at night
Mask hypoglycemia
how do nonselective beta blockers work
stimulates beta 1 (heart) and beta 2 receptors (lungs)
propranolol (Inderal) MOA & indication
nonselective beta blocker
block beta 1 & beta 2 stimulation causing decrease HR, BP, O2 demand
propranolol (Inderal) c/c
don’t stop abruptly
NO bronchospasm pts, 2/3rd degree blocks
Caution in diabetics (masks hypoglycemia), hyperlipidemia
what kind of beta blocker is carvedilol?
non selective blocker of alpha and beta receptors
carvedilol c/c
don’t stop abruptly
NO bronchopasms, hepatic impairment, 2nd/3rd degg block, diabetic, hyperlipidemia
Labetalol MOA & indc
nonselective alpha and beta receptor blocker to decrease BP, peripheral resistance
indication: HTN, pre-clampsia
Labetalol pt education
take with food to increase bioavailability
Labetalol c/c
compatible with preg AFTER 1st trimester
epinephrine MOA & indc
decreases vasodilation and permeability during anaphylaxis; relaxes smooth muscle, relieves bronchospasm, wheezing, angioedema
for: emergency type 1 allergic rxns (anaphylaxis)
epinpehrine DE
apprehensiveness, tremor, anxiety, weakness, headache, palpitations, angina, alterations in blood sugar, disorientation, hypertension
