Progesterone’s role & examples
- “anti-androgen” effect
- thicken’s & stabilizes endometrium
- thickens cervical mucus
- thins vaginal mucosa
- relaxes smooth muscle of uterus
in contraception: suppress ovulation (shuts off LH), thicken mucus
ex) implant (Nexplanon)
Levonorgestrel-releasing intrauterine device (IUD)(Mirena, Skyla) Depot medroxyprogesterone (DMPA) injection (Depo-Provera)
Progesterone SE’s
Irregular breakthrough bleeding
Breast tenderness
Galactorrhea
Nausea
progesterone contraindications
- sus preg
- sus breast cancer
- undx uterine bleeding
- liver tumor, cirrhosis, liver dz
Drug interactions with combined OC’s
NO anticonvulsants
- rifampin reduces serum EE and progestin levels
Progesterone in menopause
NEED a uterus to go on this! (same with estrogen)
Micronized Progesterone vs Medroxyprogestrone acetate (MPA)
-Micronized 1st line over MPA for protecting endometrium w/ min effects on lipids, BC, and CAD
Progesterone in menopause
IUD not approved for endometrial protection for menopause but used off label who can’t tolerate oral progestin
Mifepristone (Mifeprex, Korlym) indication
progesterone antagonist
-termination of intrauterine pregnancy, Cushing syndrome
