Features of glandular fever
Fever
Sore throat
Fatigue
Lymphadenopathy - swollen lymoh nodse
Tonsilar enlargement
Splenomegaly + splenic rupture in rare cases
What test for in glandular fever
Heterophile antibodies
How long does it take for heterophile anitbodies to be produced in response to EBV
6 weeks
What are heterophile antibodies
Multipurpose antibodies produced by the body in response to EBV
How test for EBV
Monospit test - Heterophile antibodies react ot horse RBCs
Paul Bunnell test - uses sheep RBC
When are tests for infectious mononucleosis unrelaibale
100% specific
Not everone with IM produces heterophile antibodies
Can take up to 6 weeks for ABs to be produced
ONly 70-80% sensitive

Viral capsid antigen

Rises eraly - acute infection
What does IgG antibodies for EBV show
Persists after condition
Suggests immunity
Management of glandular gever
acute ill 2-3 weeks, self limiting
Can leave with fatigue for several months once infection cleared
Advice after glandular fever
Avoid alcohol = impairs ability live to process
Avoid contact sports = risk splenic rupture
Emergency surgery if this happens
Complications with galndular fever
- Splenic rupture
- Glomerulonephritis
- Haemolytic anaemia
- Thrombocytopenia
- Chronic fatigue

Abdominal pain = pancreatitis
Testicular pain + swelling - orchitis
Confusion, neck stiffness and headache (meningtis or encephalitis)
Orchitis
Testicular inflammation

ECHo - check fro coarctation of the aorta
How diagnose mumps?
PCR on saliva swab
Also tested for antibodies to mumps virus
Management of mumps
Supportice, rest, fluids, analgesia
Self limiting
Notifiable disease
Complciations of mumps
Pancreatitis
Orchitis
Meninigtis
Sensorineural hearing loss
