What is a simple febrile convulsion?
Generalised tonic clonic seizures lasting 15 mins - occur once during single febrile illness
What is a complex febrile seizure?
Partial or focal seizures >15 mins OR occur multiple times during same febrile illness
Differentials for febrile convulsion
Epilepsy
Neurological infections eg meningitis
Intracranial space occupying lesions
Syncopal episode
Electrolyte abnormalities
Truama
Examples of neurological infections
Meningitis
Encephalitis
Cerebral malaria
How long do febrile seizures normally last?
2-5 mintutes
Mnagement of febrile convulsions
Manage fever with paracetemol/ibuprofen - simple analgesia
What advice to give parents on if a further febrile convulsion occurs?
Stay with child in sage place eg carpeted floor with pillow under head
Place in recover position, away from sources injury
Nothing in mouth
Call ambulance if >5 mins
When should go to the hospital for assessment with febrile seizure?
Have to go with first seizure
Can manage subsequent at home and visit GP at next available opportunity if confident
Risk of developing epilepsy?
- 1.8% for the general population
- 2-7.5% after a simple febrile convulsion
- 10-20% after a complex febrile convulsion
