Broad QRS complex Rapid rate Distorted Regular rhythm No P waves No T waves

Monomorphic VT
A
VT due to one source
Mainly due to presence of a scar
Wide QRS, similar shape and height

Polymorphic VT
A
Associated with Torsades de Pointes.
Prolonged QT interval
Change in height of QRS complexes (as they seem to be twisting on a point)
Cause: hypokalaemia, drug induced prolonged AP

Acute Management
A
Unstable Monomorphic: DC Cardioversion
Unstable Polymorphic / Pulseless VT: Defibrillation
Pharmacologic cardioversion (with drugs)
Adenosine has no effect in VT

A
ICD
Cardiac pacing
Anti-arrhythmic drugs are ineffective
