Vasculitis Flashcards
What does vasculitis describe?
Inflammation of the blood vessels
Various specific diseases cause vasculitis depending on the ____ of the vessel
size
What size vessels does Henoch-Schonlein Purpura refer to and what are key features?
Small vessels
Purpura (non-blanching rash)
IgA nephritis
What are common features of vasculitis?
Joint and muscle pain
Peripheral neuropathy
Renal impairment
Purpura
Necrotic skin ulcers
GI symptoms
What is purpura?
Purple-coloured non-blanching spots caused by blood leaking from the vessels under the skin
What investigations are taken for suspected vasculitis?
Inflammatory markers:
CRP and ESR usually raised
Anti-neutrophil cytoplasmic antibodies (ANCA)
What are the 4 classic features of Henoch-Schonlein Purpura?
Purpura
Joint pain
Abdominal pain
Renal involvement
What is the general treatment for vasculitis?
Corticosteroids
What should be considered with longer term use of corticosteroids?
GI and bone protection
PPI
Bisphosphonates
How do IgA nephropathy and Henloch Schonlein Purpura present similarly?
Both show IgA deposition in glomerular basement membrane on kidney biopsy
How are Henloch Schonlein Purpura and IgA nephropathy different?
HSP has purpuric rash on shins and affects other organs and joints, not just limited to glomerular basement membrane like IgA nephropathy
What size vessels does Granulomatosis with polyangitis (GPA) affect?
Small vessels
What ANCA is positive in granulomatosis with polyangitis?
c ANCA positive
What does Granulomatosis with polyangiitis cause?
Pulmonary-renal syndrome
Glomerulonephritis
Saddle shape nose
What size vessels does eosinophilic granulomatosis with polyangiitis affect?
Small
