UTI Flashcards
What distinguishes an upper UTI from a lower?
Upper includes the kidneys
Desribe ABU
Asymptomatic bacteremia that does not have sx
Gold standard for dx of UTI
culture
An urine analysis ain’t worth this?
sh*t
What things would one expect to find in a positive UA
bacteria, WBC, inflammatory cytokines
Tx for UTI
TMP-SMX - W alos Nitrofurantoin, M- FLouroquinolone
Causative agents for uncomplicated UTI?
E.coli (75%), S.Saprohyticus (5-15%)
Describe what makes a UTI complicated?
If it is caused by the use of a catheter, genetic abnormalities, obstruction, problems emptying bladder.
What percent of polynephritis is of blood origin?
10%
What virulence factors make E.coli the VIP of UTI’s?
surface adhesions faciliate binding to epithelial cells
P fimbriae - bind renal epithelial cells (polynephritis and bloodstream infections)
Type 1 pili - mediate binding to uroepithelial cells
List risk factors for uncomplicated UTI
intercourse, antibiotic use, diaphragm/spermicide use, recurrent UTI
List causes for complicated UTI.
E.coli, S.Aureus, Entercocci, yeast/fungal, Kliebassa, Pseudomonas
Triangle of pathogenesis for UTI? includes with factors?
Organism, host, enviroment
What are the 5 innate defenses against bacterial UTI
- urine flow
- urine chemistry- lysoyzmes and lactaferrin
- cell surface proteins
- normal flora
- secretory IgA
3 major source of UTI
- community acquired - GI flora
- Nosocomial - caths etc.
- Immune - Glomerulonephritis
Culture positive for gram(-) rod, MacConkey (+) and indole (+) indicates what organism causing UTI
e. Coli
Culture positive for gram (+) cocci, catalase (+), coagulase (-), novobiocin (resitant)
staph. saprophyticus
Which UTI causing organism would test indole(-)
Kleibsiella
If a man or child has a UTI think this.
complicated
young women incidence of uti per year is this
about 1 per year (0.5-0.7)
What is the difference between selective and differential media?
selective inhibits growth of type of microbe
differential indicates via dye,etc the presence of certain products.
