What is a “wheal” ?

A

Wheals - pruritic, pink or pale swellings of the superficial dermis with initial erythematous flare around them

Individual lesions come and go rapidly, by definition, generally within 24 hours

What defines acute v. Chronic urticaria

A

Acute ≤ 6 weeks

  • Reproducible and immediate reaction to allergen
  • May lead to anaphylaxis

Chronic > 6 weeks

  • Dx of exclusion
  • Recurrent over 6+ weeks
  • No obvious trigger
  • Smaller lesions
