A

Kidney disease
-glomerulonephritis, bilateral acute pyelonephritis, severe hypertension
-nephrotoxic (contrast, anesthesia, heavy metals…)
-intravascular hemolysis (destruction of red blood cells)
-large amounts of myoglobin in the circulation resulting from muscle trauma or ischemia
