presence of blanchable erythema or changes in sensation, temp, or firmness may precede visual changes
stage 1
partial thickness skin loss with exposed dermis
damage to the epidermis and part of the dermis but not through the dermis
stage 2
full thickness skin loss
damage through epidermis and dermis into the subcutaneous tissue
stage 3
full thickness skin and tissue loss
damage that extends through the epidermis, dermis, subcutaneous tissue to the exposed bone, tendon, or muscle
stage 4
obscured full thickness skin and tissue loss
ulcers covered with necrotic tissue are unstageable because the depth of tissue destruction is not visible
unstageable
persistent non blanchable deep red, maroon, or purple discoloration
skin damage that manifests as purple, blue or black areas of intact skin
signals deeper tissue damage and may need time to manifest
deep tissue injury
